File of Life

The File of Life is a free program for the citizens of Clackamas Fire designed to provide pertinent medical information to responding paramedics in the event of an emergency. This program could help save your life.

During emergencies seconds count. Often times during an emergency a patient is unable to provide critical medical information to paramedics. File of Life provides this information so life saving decisions and treatments are not delayed and the patient receives the best possible care.

The File of Life is a quad-fold card that contains information such as:

  • Patient name
  • Medical conditions
  • Medications
  • Allergies
  • Emergency contacts
  • Existence and location of a POLST (Physician Order For Life Sustaining Treatment)
  • Other vital information

The card is kept in a red plastic magnetic file folder labeled “File of Life” that attaches to your refrigerator and is easily visible to emergency responders.

Some helpful hints regarding your File of Life.

  • Use pencil to fill out your File of Life. That way any changes are easily made.
  • Keep your File of Life record up to date. Add or delete changes in medications or medical conditions as soon as possible.
  • When completed, place the File of Life on the outside face of your refrigerator.
